Health Sciences Observation Request

Use the form below to request a Mayo Clinic Health Sciences Observation. Reserve at least six weeks after submitting your request to fully complete the scheduling and onboarding process.

Before you begin:

Review the Observation Guidelines below.
Review the Compliance Training.
Complete the Confidentiality Agreement Form (parent/legal guardian signature is required if you are under 18). You will be prompted to provide it later in the process.
Complete the Observation Agreement Form. You will be prompted to provide it later in the process.
Complete the Observation Parent Consent Form (only required if you are under 18). You will be prompted to provide it later in the process.
Prepare your resume. You will be prompted to provide it later in the process.
Be prepared to provide proof of Covid-19 vaccination, or indicate that you wish to decline.

After submitting this form, you will receive a confirmation email requesting the required documentation. All documents must be received before the Program Coordinator is notified of your request.

Observation Guidelines

The goal of the Observation Program is to provide a real-world experience to help prospective Mayo Clinic School of Health Sciences (MCSHS) students confirm their program of interest is the one they wish to pursue and to help make informed decisions on education and training.

Observers are limited to one request per calendar year for a maximum of four hours.

WHAT IS AN OBSERVATION?

An observation is a 3-4 hour shadowing experience. Observers experience:

Interactions with Mayo Clinic employees.
Procedures and technology within the career field.
Skills and knowledge needed to be successful in the profession.
Individual and team contributions to the care of patients.
Mayo Clinic atmosphere and values.

EXPECTATIONS OF OBSERVERS

Observers must be a U.S. Citizen or U.S. Permanent Resident.
Observers may not request an observation to fulfill shadowing hours for a school assignment.
Observers must be interested in a MCSHS program.
Observers must be willing to commit to one half-day of shadowing. Observations are scheduled on weekdays during daytime hours.
Observers must be able to present a valid government-issued ID on their observation day. Observers will not be allowed to observe without an ID.
Observers must complete compliance training and agree to respect patient privacy.
Observers must abide by the dress code by wearing business casual clothing. Sandals, tennis shoes, leggings, and jeans are not allowed.
Observers must show respect and courteous manners during the observation, following instructions as given.
Observers are strongly encouraged to send a thank you note after the observation.
